We are a referral-based periodontic clinic and accept referrals from dentists and other dental specialists.

Dental professionals, please complete our referral form below and submit it by email or mail.

Patients, when your dentist determines that specialized treatment is required to support your oral health journey, they will provide a referral to our clinic. After receiving your referral, our team will contact you to schedule your intake appointment.
